Who we are

This website is operated by Rejuven8 Med Spa Inc, doing business as Rejuven8 Med Spa, 22339 Sherman Way, Canoga Park, CA 91303. "We", "us" and "our" mean Rejuven8 Med Spa Inc.

This policy covers personal information we collect through this website, and when you call or email us about a request made here. It does not cover patient records created during a consultation or treatment at the spa.

What we collect, and why

We collect these categories of personal information:

• Identifiers and contact details: your name, phone number and email address. We use them to answer your request and arrange your consultation
• Your request: the treatment you pick, a preferred date, and anything you write in the message box. We use them to understand what you are asking about and to plan your consultation
• Communications: your calls and emails with us and your contact preferences. We use them to keep in touch about your request
• Internet activity: pages viewed, the link or search that brought you here, campaign tags, the first page of your visit, device and browser type, and IP address. We use them to keep the site secure and working and, if you have not opted out, to measure and improve our website
• Approximate location derived from your IP address, used for the same purposes as internet activity

Where it comes from

From you, when you fill in a form, call or email us. From your device, automatically, when you use this website. From the analytics service named below, as reports about how our website is used.

The treatment you pick

When you send our form, the treatment you picked goes to our lead management system and in the email to our office, together with your contact details.

When the form is sent, we also tell Google Analytics that a form was sent, but not which treatment was picked. Your name, phone number, email and message are not sent to Google Analytics. Visiting a treatment page also tells Google Analytics which page you viewed. If you opt out, as described under "Your choices", none of this is sent.

Health information

This website is not a patient portal. Please do not use our forms or email to send medical history, medications or other health details. Your provider will ask about your health in person at your consultation.

Who we share it with

We share personal information only with service providers that help us run our business and this website, under terms that limit how they can use it:

• Our lead management system, which runs on Google's cloud services and stores the requests sent through our form
• Resend, the email service that delivers each form request to our office
• Google reCAPTCHA, which checks that a form is sent by a person and not a bot. It reads device and browser signals and is governed by Google's Privacy Policy and Terms of Service
• Google Maps, which shows our location on some pages and receives your IP address when the map loads
• Google Analytics 4, for website measurement
• Accessibilik, the accessibility toolbar on this website, which may store your display choices in your browser
• Our website host, which processes the technical data every website visit generates
• Professional advisors, and authorities where the law requires it

Booking and financing

Online booking is handled by Mangomint at booking.mangomint.com/144166. What you enter there is collected by Mangomint for us, under Mangomint's own privacy policy.

Financing through Cherry and Affirm is offered by those lenders. If you apply, your application goes to the lender under its own privacy policy.

Selling and sharing

We do not sell personal information.

This website runs no advertising pixels.

Our website is not directed to children under 16, and we do not knowingly collect personal information online from anyone under 16. A parent or guardian can contact us about a minor.

Your choices

Analytics runs from your first visit unless you opt out. Use "Opt out" on the cookie notice, or the "Cookie settings" control in the footer of any page. It shows your current setting and lets you change it in either direction. Opting out switches off the tags already running and clears the cookies they set, without a reload.

We treat a Global Privacy Control signal from your browser as a request to opt out, and apply it automatically. Browsers send "Do Not Track" in different ways and there is no common standard for it, so we rely on Global Privacy Control instead.

You can also block or delete cookies through your browser settings, though doing so may affect how parts of this website work.

How long we keep it

We keep requests sent through this website for as long as we need them to respond to you and arrange your visit, and to meet our legal and tax record-keeping obligations. Then we delete or de-identify them. If our systems fail to deliver a request, a copy may be kept for a short time in our website host's error logs so the request is not lost. Website analytics data is kept no longer than the retention set in the analytics service, and the cookies' own lifetimes are listed in our Cookie Policy.

Your privacy rights

California residents have rights under the California Consumer Privacy Act. We honor the same requests from every customer. You can ask us to:

• Tell you what personal information we have collected about you, where it came from, why we use it and who receives it
• Give you a copy of it
• Delete it, subject to exceptions the law allows, such as records we must keep
• Correct it if it is inaccurate
• Stop any sale or sharing of it

How to make a request

To use these rights, contact us at info@rejuven8md.com or call 747-230-2282. We will confirm receipt within 10 business days and respond within 45 days, and tell you if we need more time. We will not treat you differently for making a request.

To protect you, we verify a request by matching details you give us against the details we hold, such as your name, phone number and email address. You can ask someone to act for you as your authorized agent; we will ask for your signed permission and may ask you to verify your identity directly.

Security

We use reasonable administrative, technical and physical safeguards to protect personal information. No website or system is completely secure, so please do not send payment card details or health information through our forms.

Changes to this policy

When we change this policy we update the "Last updated" date above. If a change materially affects how we use information we already hold, we will tell you before it applies.
